Description
This course is based on a system of documentation that was developed while taking care of my friend who had been diagnosed with cancer in the jaw. A series of documents including forms and logs which assist the patient and/or caregiver in documenting daily care, tracking appointments, tests, procedures, surgeries, and other very important information in an organized, structured approach to be an
active and informed participant in the care and treatments being given.
This system can be managed by one person or by many, has the potential to bring a family together where family members can be involved as a team taking responsibility for different parts of the care in a specific, organized manner, as well as act as an effective communication tool between caregivers and enable more accurate recall of the care being given at home to attending professional.
