Documenting Care At Home
Welcome
The goal of “Documenting Care at Home by Valerie Falzo, RN, PLLC” is to provide easily accessible, straight forward, and basic tools and information to those caring for someone in the home; thereby assisting both the patient and the caregiver(s) in learning how to record important data, organize appointments, bills, and documents in order to streamline communication and give back to the patient and the caregiver(s) some of the control taken away by the illness.
Description
This course is based on a system of documentation that was developed while taking care of my friend who had been diagnosed with cancer of the jaw.
A series of documents including forms and logs assist the patient and/or caregiver in documenting daily care, tracking appointments, tests, procedures, surgeries, and other very important information in an organized, structured approach to be an active and informed participant in the care and treatments being given.
This system can be managed by one person or by many and has the potential to bring a family together where family members can be involved as a team taking responsibility for different parts of the care in a specific, organized manner, as well as act as an effective communication tool between caregivers and enable more accurate recall of the care being given at home to attending professionals.
What you will learn:
1) How to document the care you are giving by keeping track of such things as Medications given, Bandage changes, Tube feedings, Vital Signs (Blood Pressure, Pulse, Respirations) and other necessary information using the custom made “Daily Sheets.”
2) How to evaluate the effectiveness of pain medications by using the universal pain assessment rating tool (The Wong-Baker Faces) and how to document that effect.
3) How to use the various logs and lists to keep track of doctor appointments, special procedures, X-Rays, surgeries, overview of medications and other crucial information.
4) How to organize bills, co-pays and health insurance explanation of benefits (EOB) into 2 large 3 ring binders for easy access in order to oversee finances and health insurance benefits.
